Aftercare

Families and sons leave Capstone with a two year Finish Strong Game Plan. This game plan is a step-by-step individualized approach to completing the goals of treatment. Instead of being a punishment or a restriction of freedom, it is indeed a strategy for personal success for the graduate and therefore, a positive strategy for the family as a whole. Each graduate completes his Weekly Game Plan Booklet which includes his weekly schedule and the primary components of a successful recovery, or quite frankly, a successful life for anybody.

As the family works the Game Plan, Capstone provides the following aftercare components without a fee. This is a very strong part of our program that you should compare to others. The minimum 90 day treatment experience is imperative to success but it is important to remember that in the big picture graduation from Capstone is like getting to first base.

Going from first base to second, then third, then home plate takes approximately two years. It takes this long for the brain to heal and sort of reprogram itself. It also takes this long for the changes to become a lifestyle not just a temporary response. Most treatment programs will encourage graduates to participate in 12 step programs for their aftercare but then leave the graduate to do this on their own. While taking ownership for their lives and recovery is the goal, most graduates of residential treatment do not do the follow up. At Capstone, we want to walk along side our graduates and graduate families during the toughest sections of recovery as we gradually help them to make a complete transition to being on their own. This is called our Aftercare Program. Capstone’s Aftercare Program simply stands alone in its thoroughness and follow-through.

- Weekly 50 minute phone sessions with the resident’s Personal Therapist for 3 months after graduation.

- Two Family Reunions within the first year following graduation – therapy with Personal Therapist (graduates can come to reunions for as long as they want but there is a fee after the first year).

- One-day Family Intensive back at Capstone during the 4th month post discharge that is back-to-back with the Tune-Up day for the graduates.

- Relapse Prevention Tune Up for resident and his graduating cluster during the 4th month that runs on consecutive days with the One-Day Family Intensive (at Capstone).

- Monthly emails check-ins by the Personal Therapist for two years.

- One year comprehensive drug test kit which includes supplies for twice weekly urinalysis, alcohol breathalyzers, and adulteration testing. Included in the kit is a stamped self addressed result card to be sent to Capstone every two weeks so we can monitor the graduate’s progress.

• During treatment Capstone helps residents begin working on the 12 Steps, with God as the higher power, through Celebrate Recovery material. We strongly recommend that residents and their families continue this work with Celebrate Recovery on their own.

• During the first three months of weekly aftercare Capstone recommends that the graduate and family begin a connection with a local therapist. This transition is important to help continue progress as the graduate and family work toward a day when returning to active addiction and the drug culture is not going to be likely. Our game plan will not conflict with a qualified therapist. For some guys and/or families this is once per week for a while, others once every two weeks, and still others even less. But with almost everybody it is a help to have someone competent and physically available.
